Participation is open to all persons. Those who wish to attend the Congress should fill in the Registration form and return it to the Secretariat. Each delegate must fill in a separate form. Additional forms can be obtained from the PAQS Organizing Secretariat or forms may be photocopied.

Payment of fees must accompany all registrations. All fees must be paid in US Dollars only by VISA or bank draft drawn on a Singapore bank. Your name and full address should be typed or printed clearly on the back of the draft.
All cancellations must be notified in writing by post or email to the Congress Secretariat at the address shown. Cancellation received by 31 March 2006 will receive a full refund less a US$100 administration fee. There will be no refund for cancellation after 31 March 2006, however, a delegate substitution is most welcome.
